Tattoo Pain: What to Expect and How to Prepare

At Southernmost Tattoo in Key West, one of the most common questions we get from clients is: “How much is this going to hurt?” The truth is, tattoo pain is different for everyone. It depends on where you’re getting tattooed, your personal pain tolerance, and even your mindset going in. That said, there are certain areas of the body that tend to hurt more than others, and we want you to be informed before your appointment.

Most Painful Areas to Get Tattooed

Some spots on the body are simply more sensitive due to nerve endings, bone structure, or thinner skin. Based on both artist experience and client feedback, these areas typically rank as the most painful:

  1. Ankles & Shins – Thin skin and bone right beneath the surface make these areas extremely sensitive.
  2. Calves – Surprisingly high on the pain scale for some, especially the back of the calf where more nerves are located.
  3. Chest & Sternum – Particularly around the collarbones or center chest where bone and nerves meet.
  4. Hands & Feet – Packed with nerve endings and little padding, these spots are notoriously intense.
  5. Elbows & Knees – Both the joint and the surrounding skin can be uncomfortable due to movement and bone proximity.
  6. Ribcage – One of the top contenders for most painful, especially during long linework sessions that stretch across each rib.

Least Painful Tattoo Locations

Looking for a more tolerable spot for your first tattoo? These areas tend to be easier to handle:

  1. Outer Thigh – Lots of muscle and fat here provide cushion from the needle.
  2. Upper Arm/Outer Bicep – A common starter location with manageable sensitivity.
  3. Shoulders – Good for medium to large designs with less pain.
  4. Upper Back – Generally not too sensitive except near the spine.
  5. Forearms – Not pain-free, but most clients find them tolerable.

Factors That Affect Tattoo Pain

  • Artist Skill – An experienced artist with a light, confident hand can make all the difference.
  • Tattoo Style – Bold lines, heavy shading, and large fill areas tend to hurt more than fine line or single-pass work.
  • Body Type – People with less muscle or fat in the area being tattooed may feel more discomfort.
  • Hydration & Nutrition – Being well-fed and hydrated helps your body tolerate pain and heal better.
  • Mindset – Anxiety and tension can make pain feel worse. Breathing and staying relaxed goes a long way.

Tips to Reduce Tattoo Pain

  • Get a good night’s sleep before your appointment.
  • Eat a balanced meal 1–2 hours before your session.
  • Stay hydrated before, during, and after your appointment.
  • Avoid alcohol or blood-thinning medications beforehand.
  • Let your artist know if you need a break. We’ve got you.
